If you will be trying out for sports or participating in pre-season conditioning for the upcoming 2021-2022 school year, your physical must now be on the new form.

Any physicals submitted for the 2021-2022 school year must be completed on the new form and dated on or after May 1, 2021.  The new VHSL physical form can be found under the Links/Files tab.  Please contact the Activities Office with any questions.
