Forms Needed for Tryouts

Tryouts begin on Monday, November 10th.

All Athletes must comply with three items or they will not be allowed to try out for any winter sport including swim and dive.

1) Valid VHSL physical on file.

2) Both Parent (one parent) and Athlete must compete the Concussion component.

3) Emergency Care Form must be submitted (even if you already gave one for Fall Sport or to school clinic and files). Emergency Care Forms can be brought to Activities Swim Dive Mailbox, or to Walk In Registration.

Below is the information regarding the concussion component.

Reminder: Athletes and their parents MUST retake the concussion component every year. (If they already did it for fall sports, they are good to go.) They cannot try out if this has not been done.

Go to Announcements and click on the article “Concussion Link Now Available”.

Click on the link to the FCPS concussion power points. There is one for Parent and one for Athlete. Make sure you enter the student ID # for both. You do not need to print out a certificate. The results will automatically go to our Athletic Trainer.
