UPDATED Physical Form for AFTER 5/1/2016
USE THIS FORM for physicals you have for NEXT SCHOOL YEAR. New physicals AFTER May 1, 2016 are valid through next school year/tennis season and should be submitted to the Activities Office. DO NOT USE OLD FORMS. Your physical will not be valid and you'll have to go back for a do-over.
VHSL MATCH GUIDELINES
From the VHSL WEBSITE: regulations regarding warm up time limits, breaks between sets, restroom use, sportsmanship, acceptable communication with players on the court, defaults and penalties, etc. These regulations apply NOT ONLY at State Tournaments, but for ALL match play throughout the regular season and post season.

Student DRIVER/ Student Passenger FORM
Team members who DRIVE must complete this form if they plan to drive to and from practices/matches/tournaments/etc. and have other team members as passengers. Any team members who plan to BE a PASSENGER in another team member's vehicle must complete this form.
Driver Information FORM
ANYONE driving a vehicle with team members in it (even player driving SELF) must complete this form. If you plan to transport players to/from matches or events at any point this season, fill this out, please.
