Physical
All athletes must have a physical on file before they are allowed to tryout, complete this form and turn in to activities office before tryouts on November 13.

All athletes need to have this turned in as well to participate, please turn in by November 13.

Student driver/rider
This form must be filed with the team to ensure that program has parental permissions to drive or ride with other students. This form must be filled out and returned if not riding the bus to/from meets and practices
Driver Insurance
ANYONE who drives to or from Track related activities must have this form filed with the program to ensure out athletes are with insured drivers, or driving with insurance themselves
