Trip Participation and Driving/Riding Forms
These three forms need to be completed and returned ASAP. If you are going to drive or ride with anyone to or from a practice or a meet, we need these forms to be completed. If both an Athlete and a Parent are going to drive any other athletes, then two separate Insurance forms need to be filled out and signed. Thanks!
