VHSL Physical From
All athletes must have a physical on the official VHSL form on file in the Activities Office before conditioning and tryouts. If you had a physical done in an FCPS school this spring, it is already on file.
Emergency Care Form
All athletes must have a physical on the official VHSL form on file in the Activities Office (if you had a physical done at an FCPS school this spring, it is already on file.) You must also turn in an emergency care form to the coach.
