A current Virginia High School Sports League Physical Form (see link below for form) must be on file with the Oakton Activities Office (dated AFTER May 1, 2018) prior to tryouts
Please DO NOT SUBMIT THIS FORM directly to the Coach due to privacy issues.
Please send in (or drop off) this form to Melissa Weber in the Oakton High School Activities Office no later than Friday, July 20, 2018.
Remember: Physicals MUST be on file with OHS Athletics prior to tryouts!
FCPS Concussion Education (Athlete & Adult) Fall 2018

Each athlete AND a parent (or guardian) must complete the FCPS Concussion Education Program prior to try outs.
Please make sure both the athlete and responsible adult complete this on-line program by Friday, July 20, 2018. Once complete, your pass certificate will be electronically transmitted to OHS.

Please submit a complete, up-to-date FCPS Emergency Care Form to the OHS Activities Office (marked "golf" in upper right corner) by Friday, July 20, 2018.
Each athlete MUST provide an up-dated form, even if on the team last year.
