Participation Policy (pdf)
Every athlete must turn in a participation policy to their coaches once teams have been selected. Download and read this updated 2013-14 Extracurricular Activites Participation Policy, have athlete and parent both sign, and return to your coaches.
Emergency Care Form (pdf)
You must use this form for 2013-14 sports seasons. This form is submitted to the coach before any off-season conditioning or tryouts.
VHSL PHYSICAL
You must use this NEW VHSL physical form(Athletic Participation/Parental Consent/Physical Examination Form) for the 2013-14 sports seasons. It has Revised March 2013 in the upper right hand corner. The actual physical must be after May 1, 2013 for 13-14 fall, winter and spring sports. This completed form is turned-in to the Activities Office to Mrs. Nugent.
